Care Manager

Ready to make an impact in the community? Northeastern Vermont Regional Hospital (NVRH) is seeking a Care Manager to join the team.
The Care Manager works collaboratively with the Care Management department as well as interdepartmental health care teams and collaborates with community partners to ensure continuity of care.
The Care Manager is responsible for providing collaborative departmental support to achieve metrics for conditions of participation including utilization review, medical social work, discharge planning, crisis mental health and the NVRH Swingbed Program.
Roles and Responsibilities: Assess patients and family system to obtain information about home environment, family relationships, health history and patient’s cultural values.
When appropriate coordinate discussions with patients and/or families to facilitate understanding of medical recommendations.
Participate in daily Interdisciplinary Rounds.
Communicate with physicians and other clinical personnel in the development and implementation of patient-centered discharge plans for inpatients admitted at NVRH.
Communicate and coordinate discharge care plan in line with patient wishes with internal clinical personnel and across the continuum of care and serve as a patient advocate.

Qualifications: To be considered, candidates will need a Bachelor’s Degree in nursing, Human Services or related field.
If not currently certified you must be willing to become certified in Case Management or Care Management within two years of hire.

Schedule is M - F with shared weekend rotation.
