Job Description:
Targeted care management will be working with referred clients, and will help clients coordinate care and refer to needed services.
Targeted care manager is expected to work collaboratively with treatment team, and will work with client to create a person-centered treatment plan.
This position will help clients identify and address gaps in care and access.
*Job Qualifications:*
Applicant must have at least a Bachelor’s degree in any of the following Child and Family Studies, community mental health counseling, education, nursing, occupational therapy, psychology, recreation, and recreational therapy, rehabilitation, social work, sociology, speech and hearing; or NYS licensure and current registration as a registered Nurse and bachelor’s degree;
*AND*
Experience: Two years’ experience (a master’s degree in a related field may substitute for up to one year of experience) preferred, either:
Providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse; or Linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successfully living in a community setting.

Pay: $45,000.
